How to Get Certified at Miracle Leaf® Pooler

Before Your Appointment

Have the following ready when you arrive. This shortens your visit and helps the physician complete your certification accurately.

  • Valid government-issued photo ID. Georgia residency is required.
  • Medical records that document your qualifying condition. Include diagnostic reports and treatment history.
  • List of current medications and supplements.
  • Method of payment for clinic and state fees. A Georgia Low-THC Oil Patient Registry fee applies in addition to the clinic visit fee.
  • For minor patients: a parent or legal guardian must attend.
  • For telehealth visits: a quiet space, a working camera, and a stable internet connection.

The Certification Process

  1. Book your appointment online. Select an in-person visit at our Pooler clinic or a telehealth visit where state law permits.
  2. Complete intake paperwork. Bring the checklist above. Allow 15 to 20 minutes for forms before your physician visit.
  3. Meet with a state-licensed Miracle Leaf® physician. The physician reviews your records, discusses your qualifying condition, and confirms eligibility.
  4. Receive your certification. Your physician submits the certification to the state registry on your behalf.
  5. Complete state registration. The Georgia Department of Public Health processes Low-THC Oil registry applications within approximately 15 business days of receiving the physician certification.

Official patient information is published by the Georgia Access to Medical Cannabis Commission.
