How to Get Certified at Miracle Leaf® Lake Worth
Before Your Appointment
Have the following ready when you arrive. This shortens your visit and helps the physician complete your certification accurately.
- Valid government-issued photo ID. Driver license or state ID showing your current address. Florida residency is required. Seasonal residents qualify with proof of seasonal residency.
- Medical records that document your qualifying condition. Include diagnostic reports, treatment history, and any specialist letters.
- List of current medications and supplements. Include dosages and prescribing physician names.
- Method of payment for clinic and state fees. Miracle Leaf® accepts most major payment methods. The state fee is $77.75 in addition to the clinic visit fee.
- For minor patients: a parent or legal guardian must attend and complete caregiver registration.
- For telehealth visits: a quiet space, a working camera and microphone, and a stable internet connection.
The Certification Process
- Book your appointment online. Select an in-person visit at our Lake Worth clinic or a telehealth visit where state law permits.
- Complete intake paperwork. Bring the checklist above. Allow 15 to 20 minutes for forms before your physician visit.
- Meet with a state-licensed Miracle Leaf® physician. The physician reviews your records, discusses your qualifying condition, and confirms eligibility.
- Receive your certification. Your physician submits the certification to the state registry on your behalf.
- Complete state registration. After certification you complete your application in the Medical Marijuana Use Registry and pay the state fee of $77.75. The state processes applications within 5 to 10 business days. Temporary approval is often issued immediately.

Is recreational marijuana legal in Florida?
No. Possession of 20 grams or less is a misdemeanor (up to one year in jail, $1,000 fine). Larger amounts scale to felony charges. Amendment 3 of 2024 received 56% support but failed Florida’s 60% constitutional supermajority.
Who qualifies for Florida's medical cannabis program?
Patients certified by a qualifying physician for one of ten enumerated conditions under §381.986(2): cancer, epilepsy, glaucoma, HIV/AIDS, PTSD, ALS, Crohn’s disease, Parkinson’s disease, multiple sclerosis, or terminal illness. Plus a comparable-class clause allowing other conditions of the same kind. Chronic nonmalignant pain caused by a qualifying condition also qualifies.
What can a Florida medical patient possess?
Up to 2.5 ounces of smokable cannabis per 35-day supply window. Non-smokable forms are dispensed up to a 70-day supply. Edibles are capped at 200 mg THC per product and 10 mg THC per serving. Patients must purchase from licensed Medical Marijuana Treatment Centers (MMTCs).
Can Florida medical patients grow their own cannabis?
No. Home cultivation is prohibited under §381.986. All medical cannabis must be purchased from licensed Medical Marijuana Treatment Centers (MMTCs).
Does Florida honor out-of-state medical marijuana cards?
No. Florida does not recognize medical-cannabis reciprocity. Seasonal residents may apply for the Florida registry under the same requirements as permanent residents (renewed annually with a $75 state fee).
How do I get a Florida medical marijuana card?
Schedule a visit with a qualified physician registered in the OMMU system. The physician issues a certification through the Medical Marijuana Use Registry. Pay the $75 annual state fee. Once approved, the registry ID lets you purchase at any licensed MMTC.
